Office Procedure
All patients must be 18 years old or accompanied by an adult.
Please complete the patient information sheet from the packet you received via mail and bring your current insurance card (s) and identification card to your appointment.
Deductibles & Co-Pays
Many forms of insurance coverage require a "co-pay" and "deductible" to be collected from the patient. Our office policy is to collect your deductible and co-payment at the time of your visit at our office.
You may be charged a fee of $35.00 for NSF bank fees.
Referrals
Please bring a referral if needed. Check with your insurance company to determine if a referral is required.
Cancellation Policy
If there is any difficulty keeping the appointment, please call our office in advance so the time can be given to another patient.
You will incur a $25.00 fee for any missed appointments.
What to expect
Each new patient and patients returning for their annual visits will require the instillation of an eye drop to dilate the pupil for examination of the inside of the eye and to permit an accurate measurement for glasses. This eye drop may sting for a brief period and will decrease the eye's ability to focus for a few hours. When drops are used the examination is divided into two parts and you should expect your office visit to last approximately 1 - 2 hours.
The doctors make a sincere effort to stay on time for your appointment; however, uncontrollable factors and unexpected or complicated problems make a precise schedule almost impossible to keep. Additionally, patients with emergency problems will frequently need to be added to our daily schedule, creating unavoidable delays.
